Use this checklist to prepare your renewal submission under the Qelvea Refined Finish Index™ (RFI). Renewal is designed to confirm continued conformity — not to re-assess everything from the beginning.
Before You Start
✅ Confirm your accreditation expiry date
✅ Ensure you are renewing for the same services (or be ready to disclose changes)
✅ Gather your updated evidence before completing the form
What You Must Submit
1️⃣ Renewal Declaration Form
You will be asked to confirm:
- your accredited name and location
- services covered by your accreditation
- your current accreditation level (if known)
- that you have applied RFI principles consistently
- that you have used the RFI wording and marks correctly
- that you have not described the RFI as statutory or government-approved
✅ Complete and submit the Renewal Declaration Form
2️⃣ Updated Outcome Evidence
Provide a small, recent set of outcomes showing real performance.
✅ Recommended: 2–3 case sets per service type
Each case set should include:
- Before image
- Immediate After image
- In-Wear image (typically 7–14 days later)
Evidence should be:
- recent (current work)
- representative (typical outcomes, not best-only)
- consistent across cases
- anonymised
- unedited (no heavy filters or alterations)
Evidence Quality Checklist (Quick)
Before submitting, check:
✅ Clear lighting (no harsh shadows)
✅ Same angle where possible (before/after)
✅ In-wear image shows real-life wearability
✅ No face-identifying details if not consented
✅ Minimal or no filter use
✅ Files are labelled clearly (optional but helpful)
If Anything Has Changed
If you have made any material changes since your last assessment, disclose them in the renewal form, such as:
- change to services offered
- significant change in technique or product approach
- business name/location changes
- changes to how you reference the RFI or display marks
✅ If unsure, disclose it — clarity protects your renewal.
What Happens After Submission
After you submit:
- your renewal is reviewed against the RFI criteria
- you may be contacted for clarification (if needed)
- you’ll receive one of the following outcomes:
- Renewal Confirmed
- Conditional Renewal
- Renewal Deferred
- Renewal Not Granted
If You Miss Renewal
If renewal is not completed by the expiry date:
- accreditation lapses automatically
- licensed use must stop
- public register listing is removed
